Narrative:

On 01/20/12 at 7:56 p.m., Unit 1 was traveling on I-64 eastbound .2 miles
west of milepost 33. Unit 1 left the roadway to the left and struck the
center median guardrail. Unit 1 then crossed both eastbound lanes of
traffic and came to rest partially in the right lane of traffic. Unit 2
was traveling behind Unit 1. Unit 3 was traveling in the left lane of
traffic and drastically reduced speed to avoid striking Unit 1. Unit 2
struck Unit 1 in the rear side of the vehicle and attempted to move over to
the left lane to avoid striking Unit 1. Unit 2 then struck Unit 3 in the
right rear end of the vehicle with the front end of the vehicle. The right
rear passenger in Unit 3 suffered fatal injuries on scene. Icy conditons
contributed to the accident and numerous vehicles were off the roadway in
the vicinity of the crash.
